Before your last day, complete this checklist to make this transition as smooth as possible.
- This will ensure a future W-2 tax form is mailed correctly.
- Your final pay slip will be mailed to the address on file. If you are enrolled in direct deposit, only the pay slip will be mailed – your final paycheck will be direct deposited into your account as usual.
Confirm that your address is correct in Oracle prior to your last day worked. You're able to view and update your address and phone number in your HR profile in Oracle.
- Under the Me tab, select the Personal Information tile.
- Select the Contact Info tile.

3. Click the Pencil icon to edit your information.
4. Update your address/phone information.
5. Click the Submit button to send your changes to Human Resources.
If you are adding an additional contact point, click the Add button within the "Communication" section and/or the "Address" section, depending on which type of contact you are adding. Step-by-step instructions for adding a personal email address can be found here. Then click the Submit button to send your updates.
- Please note, you'll receive paper pay slips for the remaining paychecks until your last day. You'll continue to receive your paycheck funds as you have elected (paper or direct deposit).
- Once your termination date is submitted in Oracle, your PTO balance will reflect Zero (0) hours in the system. You may use PTO prior to your last day per your department process. Please work with your supervisor on any time off requests. Your PTO balance can still be found in UKG if you need to reference it once it is not available in Oracle.
- For those who have Vacation hours, it may be paid out. Please refer to your Union contract for details.

Property Return
All CentraCare-issued property must be returned to your supervisor. This includes the following items:
- Keys
- Employee ID badge
- Locators
- Pagers
- Phones
- Credit cards
Asset & Equipment Return (Laptop, Badge, etc.)
As part of your termination agreement, please return your CentraCare-owned laptop or computers, along with its power cords, to one of the designated locations. Monitors, laptop bags, and C-docks do not need to be returned.

Corporate Credit Card
- If you have been issued a corporate credit card, you must return the card before your last day of employment. Additionally, any pending corporate credit card transactions must be processed in Oracle before your separation date.
Mailbox and Locker
- Clear out your CentraCare mailbox, ensuring no personal or confidential materials are left behind.
- If you were assigned a CentraCare locker, it must be emptied and left in clean condition prior to the end of your last shift.
Some benefits will end the day your employment ends, and some will end at the end of the month in which your employment ends. Be aware that vendors will not be able to identify you as a separated team member until after the eligibility for the benefit plan has ended.
Changes to your benefits, such as enrolling in COBRA or selecting new coverage through another source can only be made once your current coverage has officially ended. We’ve provided additional information and resources below to help you make informed decisions about your next steps once your coverage(s) conclude.
